Students: Return to Campus Testing Requirements

The start of the spring semester is just days away and we anticipate your return to campus. Thank you for your cooperation to help mitigate the spread of the COVID-19 virus. Students need to complete the highlighted areas of the attached Outpatient Laboratory Request Form provided by St. Luke’s University Health Network and bring with you at your scheduled testing time. This information is required to receive the test. (If you need a copy of the Lab Request form, please contact COVID19@desales.edu)
For the spring 2021 semester, students are also required to complete a Quarantine and Isolation Plan. If you have not already done so, please take a few minutes to complete the plan found on MYDSU. All questions on the form must be completed or you will NOT be permitted to take the COVID-19 test at your scheduled time, attend in-class instruction, or move into the residence halls. Please discuss your personalized quarantine and isolation plans with your family in the event you test positive for COVID-19 or are exposed to someone who has tested positive.
